Abstract:

Introduction: Postauricular CUTANEOUS mastoid stulae are rare. There are only eighgt reported cases in the literature. The underlying causes of these cases were chronic suppurative otitis media (CSOM), including tuberculous otitis media. Case Report: A 62-year-old woman with right-sided discharging postauricular lesion. She had a 20-year history of ear discharge. She also had a history of 2-3 times drainage of a postauricular abscess, and from one year before, she had begun to experience discharge from behind the right pinna. She had no history of tuberculosis. On physical examination, a 1-cm x 8-mm  FISTULA with pearly white tissue at the opening was seen in the postauricular area .The surrounding skin featured a scar from the previously drained abscess. An otoscopic examination revealed the presence of an attic cholesteatoma with granulation tissue and a scarred tympanic membrane. Computed tomography demonstrated a soft-tissue density in the middle ear cavity that extended into the mastoid cavity and toward the CUTANEOUS postauricular area. The postauricular CUTANEOUS mastoid FISTULA is seen at presentation. Exploration of the right mastoid was undertaken and FISTULA closed.

Abstract:

Background and aim: Tuberculosis is one of the world’s most debilitating diseases and it is at the top of six most prevalent and dangerous infectious diseases recognized by World Health Organization. Although one out of every three people in the world is affected by Mycobacterium tuberculosis, CUTANEOUS infection is a relatively uncommon event. Here, we report a rare form of CUTANEOUS tuberculosis presented by multiple FISTULAs over the anterior chest wall.Case report: This is a 50 year old insulin-dependent diabetic man from Afghanistan who was presented with a history of multiple skin lesions over his anterior chest wall for 3 months duration. At first, there was a single non-painful, small papular lesion, which became fistulized and developed purulent discharge, later. The patient was suspected as a case of CUTANEOUS tuberculosis according to his nationality and prolonged history of unresponsiveness to outpatient antimicrobial treatment. This diagnosis approved based on skin biopsy result as it revealed multiple granulomas consisted of giant Langhans cells and the detection of numerous acid fast bacilli in the Ziehl-Neelsen staining of the skin secretions. Six weeks after treatment with anti-tuberculosis drugs, the signs of improvement were appeared and 6 months later there was no active FISTULA any more. Conclusion: CUTANEOUS tuberculosis may present itself in uncommonforms such as FISTULA. It might be seen anywhere including chest wall. Therefore, this impression should be considered in patients with prolonged skin lesions and featured by FISTULA formation.

Abstract:

Introduction: Bladder lithiasis is a common condition that accounts for 5% of all urinary calculi. (1) Predisposing factors include bladder outlet obstruction, neurogenic bladder abnormalities, as well as foreign bodies. (2) The size of the stones is rarely large and some sporadic case reports of calculi with giant dimensions have been published. In our reference we report a case of an extremely sizable calculus covering the whole vesicoprostatic lumen, from bladder dome to verumontanum accompanied by vesicoCUTANEOUS FISTULA

Abstract:

Introduction: The endoscopic fibrin glue or platelet-rich fibrin glue (PRFG) injection is an easy, safe and effective technique for the FISTULA. So far, the use of fibrin glue has been limited to selected cases.Case report: Our case is a three years old male child with a neck trauma resulting in a Esophago-CUTANEOUS FISTULA after a 3 month period of follow up we decided to use PRFG for this lesion after fine debridement of the FISTULA tract, and the surrounding fibrosed tissue twice with a one week interval. Our visit after two weeks showed complete recovery and normal general condition. A contrast study revealed complete disappearance of the lesion. Conclusions: In our case the PRFG completely resolved a long-standing FISTULA resistant to exhaustive conservative management. The treatment with PRFG has been proved to be effective in the selected cases and it seems that traumatic esophago-CUTANEOUS FISTULA may be one of these selections. Application of fibrin sealant should be considered early in the management of these difficult clinical problems.

Abstract:

Tissue deficiency accompanying failed hypospadias surgery makes further reconstruction difficult. Modified Cecil-Culp technique utilizes penile mobility to place the repair in a scrotal location for a short duration which helps to accomplish tension-free apposition using dartos and skin of the scrotum. A series of four cases is described where this technique proved beneficial despite adding a short subsequent procedure after a few months.

Abstract:

Case Description-The most important causes of abomasal FISTULAtion have been described in association with umbilical hernia and infection and trauma. A 2-month-old male lamb with an open orifice on the ventral abdomen was referred to the veterinary teaching hospital (VTH) of Lorestan University. Clinical Findings-In this case, the lamb had the abomaso-CUTANEOUS FISTULA due to unknown penetrating trauma. According to the owner's claim, the above-mentioned non-painful orifice was occurred in the ventral midline due to the rupture of abdominal swelling approximately 45 days ago. On physical examination, heart rate, respiratory rate, and body temperature were normal, but CBC showed slight neutrophilia and mild anemia. Treatment and Outcomes-After local anesthesia around the FISTULA, the abomasum adhering to the ventral muscular ring was dissected and closed. Then the muscular ring was closed in a vertical mattress pattern and subCUTANEOUS tissue and skin were sutured in the usual method. Clinical Relevance-During the FISTULA examination through the surgical operation, a muscular ring surrounded by fibrous margins was observed with partial abomasal herniation from the muscular defect. Seemed that adhesion between the abomasum and subCUTANEOUS tissue had prevented the development of diffused peritonitis. Early proper surgical intervention is useful for the treatment of trauma-induced abomaso-CUTANEOUS FISTULAe in ruminants.

Abstract:

Intestinal CUTANEOUS FISTULAs are a challenging matter in abdominal surgery. Different treatment modalities have been suggested. This is a report of two times injection of 10cc meglumine 300, containing Gelfoam particles, in duodeno-CUTANEOUS FISTULA, which led to good results. Gelfoam may be useful for the management of high output GI FISTULA.
